Forms & Required Documents for Community Partners

United Way of Charlotte County funds programs that provide health and human services to Charlotte County residents. Eligible programs must be able to demonstrate positive, measurable outcomes. Programs seeking United Way funding must work towards our mission of eliminating poverty in the focus areas of our bold goals: Financial Stability Education, and Wellness. The window for being eligible to apply for a 2017-2018 collaborative grant has closed.

For future reference, please see the pertinent documents for the 2017-2018 United Way Collaborative grant, the Board of County Commission Human Services grant, as well as the City of Punta Gorda Human Services grant.  Please see the guidelines to determine if your agency is eligible to apply - the process begins in the fall of every year.
